Flatworm - oval with ruffled margin. Whitish in color with black stripe tinged with red extending mid-dorsally and another encircling the body slightly in from the margin, but absent between tentacles. Dorsal surface with black, red, and white spots. Tentacles banded with black. Cerebral eyes forming inverted v-shaped cluster between tentacles and the anterior end of the mid-dorsal stripe
